This webinar will emphasize risk exposures associated with care transitions and the importance of efficient processes for transferring patients from the acute-care hospital (ACH) to skilled nursing facility/nursing facility (SNF/NF) and from SNF/NF to ACH, including the emergency department (ED). The presentation will highlight key elements in the care transition process and specify key features of a successful care transition process.

Dr. Watson joined Tallahassee Memorial HealthCare (TMH) in 1995 and is currently the Chief Medical Officer.  Prior to assuming this role, he was the Inpatient Director for the Family Medicine Residency Program, Medical Director for the TMH hospitalist group and the original Education Director for Internal Medicine at Florida State University, College of Medicine.  He graduated from Indiana University School of Medicine and is Board Certified in Internal Medicine. Dr. Watson also has experience in private practice and was the Chief Resident at Orlando Regional Medical Center. 

Lauren Faison currently works with Tallahassee Memorial Healthcare (TMH) as the Service Line Administrator for Regional Development, Population Health, and Telemedicine. In that role, Lauren oversees the Tallahassee Memorial Healthcare’s Transition Center, which provides post-acute services to high risk patients. Lauren is deploying the use of telemedicine and telehealth solutions throughout the TMH system to increase patient access, provider efficiencies and reduce unnecessary readmissions and visits to the emergency room.
